Both documents are fillable PDFs. Please ensure that you download and save both documents to your own computer before beginning to add information. We strongly recommend that you save the organizer periodically so that you do not lose any information you have entered. When complete, upload both documents by clicking on the "upload files" button above.

If you would prefer to fill in the information by hand, no problem. Please download both documents, print them, and fill them in by hand. You’re welcome to schedule a time to meet with me at my office to drop off your information and discuss any questions.
